Withdraw forms must be received no later than the 15th of your final month. (For example: if your last month is October, form must be submitted by October 15)
If this form is not received by the 15th of the month, MAP fees will have already processed, and you will be dropped at the end of the next month.
Please note, in the event a current user withdraws and re-enrolls, they will then be charged the Administrative Fee upon re-enrollment no more than 2x per calendar year.
