Withdraw forms must be received no later than the 15^th of your final month. (For example: if your last month is October, form must be submitted by October 15)

If this form is not received by the 15^th of the month, MAP fees will have already processed, and you will be dropped at the end of the next month.

Please note, current MAP Users (those enrolled before January 1st, 2025) will not be charged the $35 Administrative Fee until 2026. In the event a current user withdraws and re-enrolls, they will then be charged the Administrative Fee upon re-enrollment.
