Swim Team Withdrawal Form

To withdraw from the swim team (or place your account on hold), the below form must be received no later than the 15^th of swimmer’s final month. (For example: if the swimmer’s final month is October, the withdrawal form must be submitted by October 15 to not be charged November dues.)

If this form is not received by the 15^th of the month, your account will be charged for the upcoming month.
